Medical Social Worker

ANX Home Healthcare’s Medical Social Workers serve as conduits for patient access to community resources for counseling, including individual and family therapy, as well as determine needs and requirements for alternative living arrangements for the elderly such as assisted living and nursing facilities.

Our Medical Social Workers work with our interdisciplinary team to ensure that our patients have the resources they need to remain safely at home, as well as evaluate any psychological, social and/or medical needs that may require additional services.
